VOLUNTEER AND BEREAVEMENT COORDINATOR
We are seeking a Volunteer and Bereavement Coordinator to join our Holistic Team. The successful candidate will be responsible for the development, supervision, and implementation of the Volunteer Program. This position is responsible for meeting the volunteer needs of the organization, including support for the Bereavement program.
A qualified individual must possess critical thinking skills and be able to work independently while maintaining a professional, positive attitude. Computer proficiency and telephone experience is a must as well as good oral and written communication skills. We offer a flexible schedule, recently revised salary and benefits package, and most staff are eligible for an agency-provided vehicle.

A Manageable On-Call Schedule

Our RN Case Managers participate in:
- One evening of on-call each week
- Every fourth weekend on-call rotation
- Pager compensation while on-call
- Premium compensation for patient visits completed while on call

What You’ll Do
As an RN Case Manager, you’ll coordinate and provide skilled nursing care to patients in the comfort of their homes while working collaboratively with physicians and an interdisciplinary team.
Responsibilities include:
- Performing comprehensive patient assessments
- Developing and implementing individualized care plans
- Providing patient and caregiver education
- Medication management
- Coordinating care with physicians and other healthcare professionals
- Maintaining accurate clinical documentation
- Participating in interdisciplinary team meetings
- Delivering compassionate, high-quality home health and hospice care
Who We’re Looking For
We’re looking for compassionate nurses who enjoy building relationships, thinking independently, and making a meaningful difference in patients’ lives.
Ideal candidates will have:
- A current Michigan Registered Nurse (RN) license (or eligibility to obtain one)
- A valid driver’s license and acceptable driving record
- Strong communication, assessment, critical thinking and organizational skills
- A passion for providing compassionate, patient-centered care
- The ability to work both independently and as part of a collaborative team

Float/Team Nurse
We are seeking an exceptional RN for our Home Health & Hospice programs. Candidate must be reliable, work efficiently and possess critical thinking skills. Nursing experience and computer charting experience preferred.

Private Duty Openings
Aides
We are seeking compassionate, caring individuals to be Private Duty Aides and help area seniors be able to remain in their homes by providing personal care, homemaking, and respite/companion care. No experience necessary. Paid training and orientation is provided.
We offer competitive wages and flexible scheduling to accommodate class schedules, childecare needs, etc. An additional $3.20 per hour is paid monthly for services provided to MI Waiver clients. There are also sign on bonuses available, premium pay for holidays worked, referral bonuses for new employees sent our way, paid travel time, employee recognition, and milestone bonuses.
